2. Description of Services
Elevate Home Care MI LLC is a home care agency in Michigan that works with state-funded home care programs to coordinate approved in-home care hours. We manage approved hours, coordinate and match vetted caregivers, and handle the associated scheduling and paperwork. In-home support services we coordinate may include personal care (such as bathing, dressing, and grooming), companionship, meal preparation, light housekeeping, medication reminders, mobility assistance, and transportation to appointments.
These services are non-medical in nature. Our caregivers do not provide medical advice, diagnosis, or treatment. Care is scheduled flexibly around each client's approved hours, from a few hours per week to around-the-clock care.
3. Use of Website
You agree to use the Site only for lawful purposes and in a manner that does not infringe the rights of, or restrict or inhibit the use of the Site by, any third party. You agree not to attempt to gain unauthorized access to any part of the Site, interfere with its operation, or use the Site to transmit harmful or unlawful content.
The information on our Site is provided for general informational purposes only and may be updated from time to time. We reserve the right to modify or discontinue parts of the Site at any time without notice.
4. Client Responsibilities
Clients and their authorized representatives are responsible for providing accurate and complete information about care needs, medical conditions, medications, and relevant circumstances so that we can plan and deliver appropriate services.
Clients are expected to maintain a reasonably safe home environment for caregivers and to communicate any changes in health or circumstances that may affect care in a timely manner.
5. Care Services Disclaimer
Elevate Home Care MI LLC coordinates non-medical home care through an agency model. Caregivers who deliver in-home support are vetted, matched care providers, not licensed medical professionals, and do not provide nursing, medical, or therapeutic care. Nothing about our coordination or the information on our Site constitutes medical advice, diagnosis, or treatment.
Our office coordinates approved in-home care hours and matches caregivers to clients, and in-home care is delivered by caregivers in the home environment. You should always consult with a qualified physician or healthcare provider regarding any medical concerns, medication changes, or health decisions. In an emergency, call 911 or go to the nearest emergency room.
6. Intellectual Property
All content on the Site, including text, graphics, logos, images, and the overall design, is the property of Elevate Home Care MI LLC or its licensors and is protected by applicable copyright and intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from Site content without our prior written consent.
7. Limitation of Liability
To the fullest extent permitted by law, Elevate Home Care MI LLC and its caregivers, officers, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or data, arising out of or related to your use of the Site or our services.
Our aggregate liability for any claim arising from the Site or services shall not exceed the amount paid by you, if any, for the services giving rise to such claim, or one hundred dollars ($100), whichever is greater, in jurisdictions where a monetary limit is applicable.
8. Disputes / Governing Law
These Terms shall be governed by and construed in accordance with the laws of the State of Michigan, without regard to its conflict-of-law principles. Any dispute arising out of or relating to these Terms, the Site, or our services shall be subject to the exclusive jurisdiction of the state and federal courts located in Michigan.
